Why Lower Back Pain Keeps Coming Back

Most people who come into our Pawleys Island office with lower back pain have had it before. It eased off for a while, then came back after a long drive, a weekend of yard work, or nothing obvious at all. That pattern of settling and returning is the part worth paying attention to.

Pain quiets down before the cause does

Back pain often eases within a couple of weeks whether or not anything is done about it. Inflammation settles, muscles stop guarding, and the sharp edge comes off. That feels like recovery, but the joint restriction, disc irritation or movement habit that started it can still be there. The next demand on the back finds the same weak point.

The sore spot is not always the source

The muscles that ache are frequently doing extra work to protect something else. When a spinal joint loses normal motion, the muscles around it tighten to limit movement, and nearby segments take on load they were not built for. Treating only the tight muscle can bring short-term relief while leaving the mechanics unchanged.

This is also why pain can show up away from the actual problem. An irritated nerve root in the low back can be felt in the buttock, the thigh or down the leg, with the spine itself feeling relatively quiet.

Common patterns behind recurring low back pain

  • A spinal joint that has lost its normal range of motion
  • A disc that is bulging, dehydrated or under uneven pressure
  • Hips and mid-back that have stiffened, pushing extra movement into the lower back
  • Long periods of sitting, which loads the lumbar discs more than standing does
  • Core and gluteal muscles that have stopped carrying their share of the work

Why rest on its own tends to disappoint

Rest calms an angry back, and in the first day or two that is reasonable. Beyond that, extended inactivity allows stiffness to set in and the supporting muscles to lose conditioning, which can leave the back less able to handle normal demands than it was before. Gentle, progressive movement generally does more for a recovering back than waiting it out.

When it is worth getting it looked at

A single episode that clears up and does not return may not need much. It is more worth investigating when pain keeps coming back in the same place, when it travels into the buttock or leg, when numbness or tingling is involved, when it wakes you at night, or when each episode arrives a little sooner than the last.

The aim of an exam is to work out which structure is actually involved and why it keeps being provoked, rather than to chase the sore spot. That usually means looking at how the spine moves, where motion is missing, and what the surrounding joints and muscles are doing to compensate.

If your lower back pain keeps returning, Dr. Neil Fico offers a new patient consultation, exam and X-rays if clinically indicated for $49. Call (843) 979-CARE (2273) or book through the website.
